Improve Box File Request to include more fields and larger image uploads for the theme.
Today, I can only add one File Description field and also my image at the top (for the theme) can only be 1.5 MB, which is an extremely tiny image file.
Converting File Request into a fully functional SurveyMonkey or Google Forms competitor would be awesome!
